Why this applies to Bendamustine
Bendamustine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Eurycoma Longifolia interacts with.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP1A2 enzymes. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for tongkat ali and your bendamustine. Bendamustine is partly broken down by a liver enzyme called CYP1A2. In a test tube, an extract of this herb appears to weakly block that enzyme. If that held true in people, your body might clear bendamustine a little more slowly, leaving slightly higher levels around. With this kind of chemotherapy, that could mean more of the usual side effects, things like low blood counts, nausea, fatigue, or a higher infection risk.
The honest part: this is theoretical, based only on lab data, and it has never been shown in real patients. Still, because bendamustine is serious chemo, I wouldn't add any supplement without first clearing it with your oncologist or pharmacist.
