Tapentadol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Eurycoma Longifolia interacts with.

Theoretically, Eurycoma longifolia might increase levels of CYP2C19 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP2C19 enzymes. This effect has not been reported in humans.

Here's the short version on combining tongkat ali with tapentadol (Nucynta). The idea on file is that this herb might weakly slow down a liver enzyme called CYP2C19, which plays a small role in clearing tapentadol. If that happened, a little more of the drug could linger in your system, which in theory could nudge up its usual effects like drowsiness, dizziness, nausea, constipation, or slowed breathing.

That said, I want to keep this in perspective. This is purely theoretical, seen only in a lab dish, never in real people, and tapentadol is mostly cleared by a different pathway anyway. So a major problem is unlikely. Still, opioids deserve respect, so check in with your pharmacist or doctor before pairing them.

About Tapentadol

What Is Tapentadol Used For?

Brand names: Nucynta, Nucynta ER

Tapentadol tablets (including Nucynta and Tapentadol Hydrochloride tablets) are used for severe acute pain in adults when other treatments are not adequate. Nucynta tablets are also approved for acute pain in children aged 6 and older who weigh at least 40 kg.

Nucynta ER and tapentadol extended-release tablets are used for severe, ongoing pain that requires around-the-clock opioid treatment and cannot be managed with other options, including severe neuropathic pain (nerve pain) caused by diabetic peripheral neuropathy in adults. Extended-release formulations are not intended for as-needed use.
