Proguanil falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Eurycoma Longifolia interacts with.

Theoretically, Eurycoma longifolia might increase levels of CYP2C19 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP2C19 enzymes. This effect has not been reported in humans.

Here's a wrinkle worth knowing: proguanil is actually a prodrug. Your body uses an enzyme called CYP2C19 to turn it into its active form, cycloguanil, which is the part that actually fights the malaria parasite. So this one works backwards from what you might expect.

Lab studies hint that Eurycoma longifolia (tongkat ali) may weakly slow that same CYP2C19 enzyme. In theory, that could mean a little less proguanil gets activated, slightly reducing its protective punch. But honestly, this is only test-tube data and has never been seen in real people, so I wouldn't lose sleep over it. Still, since proguanil is protecting you from malaria, it's smart to mention the supplement to your doctor or pharmacist before a trip.
