Why this applies to Gliclazide

Gliclazide falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Eurycoma Longifolia interacts with.

Theoretically, Eurycoma longifolia might increase levels of CYP2C19 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP2C19 enzymes. This effect has not been reported in humans.

Here's the simple version: your body uses an enzyme called CYP2C19 to help clear gliclazide (Diamicron) out of your system. In lab dish studies only, tongkat ali (Eurycoma longifolia) appears to weakly slow that same enzyme. If that were to happen in real life, gliclazide could hang around a little longer and work a bit harder, which mainly raises the chance of your blood sugar dropping too low.

Now the reassuring part: this is purely theoretical and has never actually been seen in people, and it's rated as a minor concern. So I wouldn't lose sleep over it. Still, watch for signs of low blood sugar (shakiness, sweating, dizziness, confusion), and check with your pharmacist or doctor before adding this supplement.
