Nelfinavir Mesylate falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Eurycoma Longifolia interacts with.

Theoretically, Eurycoma longifolia might increase levels of CYP2C19 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP2C19 enzymes. This effect has not been reported in humans.

Here is the gist for your HIV medicine. Your liver uses an enzyme called CYP2C19 to help process nelfinavir. In lab dish testing, tongkat ali appears to weakly slow that enzyme down. In theory, that could let a little more nelfinavir build up in your blood, which might mean more of its usual side effects like diarrhea, nausea, gas, or stomach upset.

The important thing to know: this is only a theoretical concern from test-tube work, and it has never actually been seen in real people. The effect, if any, looks small. Still, because keeping your HIV treatment steady really matters, I would not add tongkat ali without first checking with your pharmacist or HIV provider so they can review your full regimen.
