Perampanel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Eurycoma Longifolia interacts with.

Theoretically, Eurycoma longifolia might increase levels CYP1A2 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP1A2 enzymes. This effect has not been reported in humans.

Here's the gist for perampanel (Fycompa). Your body uses a liver enzyme called CYP1A2 as one route to break this medicine down. In lab dish studies only, tongkat ali (Eurycoma longifolia) appeared to mildly slow that enzyme. If that were to happen in people, perampanel could clear a bit slower and drift slightly higher, which might mean more of its usual effects like drowsiness, dizziness, trouble with balance, or mood and irritability changes.

That said, this is purely theoretical and has not been seen in actual humans, and the interaction is rated minor. I wouldn't lose sleep over it, but since perampanel already causes sleepiness, it's worth a quick chat with your pharmacist or doctor before adding this supplement.

Here's the situation with tongkat ali (Eurycoma longifolia) and your Fycompa (perampanel). In the test tube, this herb weakly slows down an enzyme called CYP2C19, which is one of the pathways your body uses to clear perampanel. In theory, if that enzyme is slowed, a bit more perampanel could linger in your system, which might nudge up its effects and side effects like dizziness, drowsiness, trouble with balance, or mood and irritability changes.

The important part: this is purely a lab-based, theoretical concern. It has never actually been shown to happen in people, the rating is minor, and perampanel is not a prodrug, so the direction is straightforward. Still, since seizure medicines need steady levels, it's worth a quick chat with your pharmacist or neurologist before combining them.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
