Here's the simple version: perampanel already tends to make people drowsy, dizzy, or unsteady on their feet, especially when you first start it or after a dose increase. Guarumo, in an animal study, seemed to calm the nervous system and cause some muscle relaxation and clumsiness. So the worry is that stacking the two could add up and leave you feeling more sleepy, foggy, or wobbly than usual.

I want to be honest with you about the evidence though. This is based only on lab and animal work, so it's more of a theoretical caution than a proven problem. Perampanel isn't a prodrug, so guarumo isn't changing how it's activated, it's just a possible piling-on of sedation. Before combining them, run it by your pharmacist or doctor, and go easy with anything that needs full alertness, like driving.

Guarumo (Cecropia obtusifolia) is a tropical tree used in Latin American traditional medicine, most often as a leaf tea for diabetes and high blood pressure. Some early laboratory and small human studies suggest it may modestly affect blood sugar, but the evidence is limited and not strong enough to recommend it as a treatment. Talk with your pharmacist or doctor before using it, especially if you have a chronic condition or take medication.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
