Here's the simple version: tramadol already works on your brain and nervous system to ease pain, and it can leave you feeling drowsy, dizzy, or foggy. Guarumo, the leaf tea some people use for blood sugar, showed a calming, sedative-like effect in an animal study. Put the two together and there's a chance the sleepy, slowed-down feeling could stack up.
I want to be honest about how thin this evidence is. This concern comes only from animal research, not from people, so it's more of a theoretical caution than a proven problem. Still, if you notice extra drowsiness, lightheadedness, or trouble concentrating, go easy and loop in your pharmacist or doctor before combining them.

Guarumo (Cecropia obtusifolia) is a tropical tree used in Latin American traditional medicine, most often as a leaf tea for diabetes and high blood pressure. Some early laboratory and small human studies suggest it may modestly affect blood sugar, but the evidence is limited and not strong enough to recommend it as a treatment. Talk with your pharmacist or doctor before using it, especially if you have a chronic condition or take medication.

Tramadol treats pain that is severe enough to need an opioid pain medicine when other treatments are not enough. Other options, such as non-opioid pain relievers, should have failed, not been tolerated, or be unlikely to work.
The use depends on the form. Tramadol tablets and oral solution are for adults with pain severe enough to need an opioid. Extended-release tablets and capsules are for severe, persistent pain that needs around-the-clock, long-term opioid treatment. They are not for pain taken only as needed.
