Here's the gist: metoclopramide can already make some people feel drowsy, tired, or foggy on its own. Guarumo, the leaf tea used in some Latin American traditions, showed a calming, muscle-relaxing effect on the nervous system in an animal study. So the worry is simply that the two together could add up and leave you feeling more sleepy or sluggish than expected.
I want to be honest about the strength of this: it's based only on lab and animal work, not human studies, so it's more of a theoretical caution than a proven problem. Still, if you take metoclopramide and want to try guarumo, run it by your pharmacist or doctor first, and don't drive or operate machinery until you know how the combo affects you.

Guarumo (Cecropia obtusifolia) is a tropical tree used in Latin American traditional medicine, most often as a leaf tea for diabetes and high blood pressure. Some early laboratory and small human studies suggest it may modestly affect blood sugar, but the evidence is limited and not strong enough to recommend it as a treatment. What Is Metoclopramide Used For?
Brand names: Reglan, Metoclopramide Injection, Octamide
Metoclopramide is used in adults for two main digestive problems. The first is symptomatic, documented gastroesophageal reflux that hasn't improved with conventional therapy (oral tablets, orally disintegrating tablets and oral solution). The second is acute and recurrent diabetic gastroparesis, where the stomach empties too slowly.
Gimoti nasal spray is approved only for diabetic gastroparesis in adults. Metoclopramide injection is used for diabetic gastroparesis, preventing vomiting from emetogenic (vomiting-causing) cancer chemotherapy, and preventing postoperative nausea and vomiting. It is also used to help place a tube into the small bowel and to speed barium movement for X-ray exams.
