Evodia might reduce the levels and clinical effects of CYP1A2 substrates through induction of CYP1A2.
Evodia extract and the evodia constituent rutaecarpine induce hepatic CYP1A2 enzyme activity. Evodia decreases levels of theophylline and caffeine, CYP1A2 substrates, by about 70% in animal models.

Here's the gist for Fycompa (perampanel). Your liver uses an enzyme called CYP1A2 to help process this seizure medicine. Lab and animal studies suggest Evodia (specifically a compound in it called rutaecarpine) can speed that enzyme up. When the enzyme works faster, perampanel may get cleared from your body more quickly, which could lower its levels and weaken how well it controls your seizures.

For a seizure medicine, that's worth taking seriously, since breakthrough seizures are a real concern. That said, this is based on test tube and animal research, not solid human studies, so the real-world effect in people isn't proven. Please don't change anything on your own. Just check with your pharmacist or doctor before pairing Evodia with your perampanel.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the gist for Fycompa (perampanel): your liver relies on an enzyme called CYP3A4 to break it down and clear it out. Evodia, in test-tube studies, appears to slow that same enzyme. If that were to happen in your body, perampanel could build up a bit more than expected, which might mean stronger effects and more side effects like dizziness, drowsiness, trouble with balance or coordination, or mood and irritability changes.

I want to be straight with you, though: this is only seen in lab dishes, not in real people, so it's a theoretical concern, not a proven one. Perampanel is not a prodrug, so the direction (more drug, not less) holds. Still, since it acts on the brain, please run it by your pharmacist or doctor before combining them.

Human evidence for these uses is very limited, and it is mostly studied in laboratory and animal research. What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
