Evodia and Ciprofloxacin

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Evodia interacts with whole categories of medication. Ciprofloxacin falls into 2 of those categories, so it can interact with Evodia in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS

1 of 2

Why this applies to Ciprofloxacin

Ciprofloxacin falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Evodia interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and clinical effects of evodia.
Animal research shows that concomitant administration of ketoconazole, a known CYP3A4 inhibitor, with the alkaloid constituents of evodia significantly increases the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the thing to know with Cipro and evodia: this one is about how evodia gets cleared from your body, not Cipro itself. Ciprofloxacin can block a liver enzyme called CYP3A4, which is one of the routes your body uses to break down some of evodia's active compounds. If that pathway slows down, those evodia compounds may build up a bit higher and stick around longer.

In plain terms, taking Cipro alongside evodia could make the evodia hit harder than expected. I want to be honest though: this is based on lab and animal studies, not on people, so it's more of a theoretical caution than something proven. Since evodia itself isn't well studied, the safest move is to mention it to your pharmacist or doctor before combining the two.

CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS

2 of 2

Ciprofloxacin also falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS.

Theoretically, drugs that inhibit CYP1A2 might increase the levels and clinical effects of evodia.
The evodia constituent rutaecarpine is metabolized by CYP1A2.

Here's the thing to know about pairing Evodia with ciprofloxacin (Cipro): cipro is a known blocker of a liver enzyme called CYP1A2. Evodia contains a compound called rutaecarpine that gets cleared by that same enzyme. So in theory, taking cipro alongside evodia could slow down how your body breaks down the supplement, letting evodia levels climb higher than expected. The concern here is more side effects from the supplement, not from your antibiotic.

I'll be straight with you though: this is only based on lab and animal research, not real people, so it's quite theoretical. Still, while you're on a course of cipro for an infection, the simplest move is to hold off on evodia. Please run it by your pharmacist or doctor first, and don't change anything with your antibiotic on your own.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Ciprofloxacin page

“A glass of apple or grapefruit juice can sharply cut how much ciprofloxacin (Cipro) your body absorbs, because these juices block a transporter the antibiotic needs to reach the bloodstream. That is one of eight major-rated risks among the 163 supplement and herbal ingredients on file, and I treat this list as a genuine warning rather…”

About Ciprofloxacin

What Is Ciprofloxacin Used For?

Brand names: Cipro, Ciprobay, Cipro XR, Ciloxan, Cipro IV, Otiprio

Ciprofloxacin belongs to a class of antibiotics called fluoroquinolones. It's prescribed to treat bacterial infections in many parts of the body — including the urinary tract, skin, lungs, bones, joints, and digestive system. It's also used to protect against or treat anthrax and plague after exposure.

Because ciprofloxacin carries real risks, it's reserved for milder conditions like sinus infections, simple bladder infections, and flare-ups of chronic bronchitis only when there are no safer alternatives available.
