Perampanel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ephedra interacts with.

Theoretically, ephedra might decrease levels of drugs metabolized by CYP1A2.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.

Here's the gist for Fycompa (perampanel). Your liver partly relies on an enzyme called CYP1A2 to handle this seizure medicine. The concern with ephedra is that it may speed up that enzyme, which in theory could clear perampanel out of your system faster and leave less of it working. If perampanel levels drop too low, seizure control can slip, which is the opposite of what you want.

I want to be honest about the evidence though: this is theoretical, based only on animal and lab studies, so we don't really know how strongly it plays out in people. The bigger issue is that ephedra is banned in U.S. supplements because it can dangerously raise blood pressure and heart rate. Please talk with your pharmacist or doctor before mixing the two.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
