Perampanel falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group English Ivy interacts with.

Theoretically, English ivy might increase the levels of drugs metabolized by CYP2C19.
In vitro research shows that English ivy inhibits CYP2C19 enzymes. So far, this interaction has not been reported in humans.

Here's the idea behind this pairing. English ivy extract has shown, in lab dish studies only, that it can slow down a liver enzyme called CYP2C19. Perampanel (Fycompa) is partly handled by that same enzyme. So in theory, if the enzyme is working slower, your body might clear perampanel a little less efficiently, letting levels creep up. With more drug hanging around, you could notice stronger side effects like dizziness, drowsiness, trouble with balance, or mood and irritability changes, which this seizure medicine is already known for.

I want to keep this in perspective, though. This is only test-tube evidence, and it has never actually been reported in people taking these together. It's worth a quick mention to your pharmacist or neurologist, but no need to panic.

English ivy leaf extract is most commonly used in cough and cold syrups to help loosen mucus and ease coughing, and it is a popular ingredient in many European herbal products. Some research suggests it may help with certain coughs, but the evidence is still limited and mixed. What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
