These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Eurycoma Longifolia interacts with whole categories of medication. Warfarin falls into 2 of those categories, so it can interact with Eurycoma Longifolia in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Minor).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 2Why this applies to Warfarin
Warfarin falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Eurycoma Longifolia interacts with.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP1A2 enzymes. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for warfarin and tongkat ali. In a lab dish, this herb very weakly slowed down an enzyme called CYP1A2, which is one of several enzymes your body uses to clear warfarin. In theory, slowing that enzyme could let a little more warfarin linger in your system, which might nudge your blood thinning a touch higher. With warfarin, even small shifts matter, since too much can lead to easy bruising, nosebleeds, or more serious bleeding.
That said, this is only a test-tube finding and has never been seen in real people. So I wouldn't lose sleep over it, but because warfarin is so dose-sensitive, please check with your pharmacist or doctor before adding this supplement, and keep up with your regular INR blood tests.

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES
2 of 2Warfarin also falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP2C19 enzymes. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for warfarin and tongkat ali. In lab dishes (not in actual people), this herb has been shown to slightly slow down a liver enzyme called CYP2C19, which is one of the pathways your body uses to break warfarin down. If that slowdown happened in real life, warfarin could linger a bit longer and its blood-thinning effect could creep up, which means a higher chance of bruising or bleeding.
That said, this is purely theoretical and the effect seen was weak, with nothing reported in humans. Still, warfarin is a finicky drug where even small shifts matter, so I wouldn't add tongkat ali without checking with your doctor or pharmacist first. They may want a quick INR check if you do.

Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.

- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

About Warfarin
What Is Warfarin Used For?
Brand name: Warfarin
Warfarin sodium tablets and Jantoven tablets are both used to prevent and treat venous thrombosis (blood clots in the veins) and pulmonary embolism (a clot in the lungs). They are also used to prevent clot-related complications in people with atrial fibrillation (an irregular heart rhythm) or who have had a heart valve replaced.
In people who have had a heart attack, warfarin is used to reduce the risk of death, another heart attack, stroke, or other clotting events. It does not dissolve an existing clot, but it helps prevent the clot from growing and stops new dangerous clots from forming.
