Acetaminophen, Caffeine, Codeine, Salicylamide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Eurycoma Longifolia interacts with.

Theoretically, Eurycoma longifolia might increase levels CYP1A2 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP1A2 enzymes. This effect has not been reported in humans.

In this combo pill, the piece that interacts with tongkat ali is the caffeine. Your body clears caffeine using a liver enzyme called CYP1A2, and lab (test tube) studies hint that Eurycoma longifolia may weakly slow that enzyme down. If that happened in real life, caffeine could hang around a bit longer, so you might feel more jittery, have trouble sleeping, notice a faster heartbeat, or feel a little wired.

Here's the honest part: this is only a theoretical concern. It has never actually been shown in people, and the effect seen in the lab was weak. The codeine and acetaminophen parts aren't really affected by this. Still, if you tend to be caffeine-sensitive, mention it to your pharmacist or doctor before combining them.
