Why this applies to Warfarin

Warfarin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sweet Annie interacts with.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the honest picture with warfarin and Sweet Annie. Warfarin is a blood thinner with a very narrow safety window, so even small shifts in its levels matter. The active part of Sweet Annie (artemisinin) can affect a liver enzyme called CYP3A4 that helps break warfarin down. The tricky thing is the lab data points in two directions: one Sweet Annie compound seems to speed that enzyme up, while the whole-plant extract seems to slow it down.

Because the signals conflict and this is only test-tube and animal research, we honestly can't predict which way your INR would move. A drop could mean more clot risk; a rise could mean easier bruising or bleeding. Please loop in your pharmacist or doctor before combining them.

What Is Warfarin Used For?

Brand name: Warfarin

Warfarin sodium tablets and Jantoven tablets are both used to prevent and treat venous thrombosis (blood clots in the veins) and pulmonary embolism (a clot in the lungs). They are also used to prevent clot-related complications in people with atrial fibrillation (an irregular heart rhythm) or who have had a heart valve replaced.

In people who have had a heart attack, warfarin is used to reduce the risk of death, another heart attack, stroke, or other clotting events. It does not dissolve an existing clot, but it helps prevent the clot from growing and stops new dangerous clots from forming.
