Pentamidine falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Eurycoma Longifolia interacts with.

Theoretically, Eurycoma longifolia might increase levels of CYP2C19 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP2C19 enzymes. This effect has not been reported in humans.

Here's the gist for combining tongkat ali with pentamidine. Lab studies hint that this herb may weakly slow down a liver enzyme called CYP2C19, which helps clear certain medications. If pentamidine is partly handled by that enzyme, the theory is that levels could nudge a little higher, which might mean a slightly greater chance of its known side effects like low blood pressure, low blood sugar, kidney irritation, or heart rhythm changes.

That said, I want to be honest with you. This is purely theoretical, based only on test-tube data, and it has never been shown to happen in real people. Pentamidine is also given by injection in a medical setting where you're monitored. Still, mention any supplements to your care team before starting treatment.

Pentam 300 (pentamidine isethionate for injection) is indicated for the treatment of pneumonia caused by Pneumocystis carinii (PCP), a potentially life-threatening lung infection that most commonly affects people with weakened immune systems.

Because of its serious risks, Pentam 300 should only be given to patients in whom Pneumocystis carinii infection has actually been confirmed.
