Pentamidine falls under ANTIBIOTIC DRUGS — the group Lactobacillus Acidophilus interacts with.

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.

Here's the thing to keep in mind: this interaction runs in one direction only, and it works against the probiotic, not your medicine. Pentamidine won't be weakened or made stronger by Lactobacillus acidophilus. Your treatment for that Pneumocystis pneumonia should keep working just as it should.

The concern is the other way around. Pentamidine is an antimicrobial, and probiotics like L. acidophilus are live, friendly bacteria. If the drug reaches them, it can kill off some of those organisms and waste your probiotic dose. Worth noting, pentamidine is usually given as an injection or inhaled, so direct contact in the gut is less of an issue. This is theoretical, based on how these work, not strong proof. If you take both, spacing them out and checking with your pharmacist is a smart move.

Pentam 300 (pentamidine isethionate for injection) is indicated for the treatment of pneumonia caused by Pneumocystis carinii (PCP), a potentially life-threatening lung infection that most commonly affects people with weakened immune systems.

Because of its serious risks, Pentam 300 should only be given to patients in whom Pneumocystis carinii infection has actually been confirmed.
