Pentamidine falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Schisandra interacts with.

Theoretically, schisandra might increase the levels and clinical effects of CYP2C19 substrates.
In vitro research shows that schisandra inhibits CYP2C19, and animal research shows that schisandra increases the concentration of voriconazole, a CYP2C19 substrate. Theoretically, schisandra may also inhibit the metabolism of other CYP2C19 substrates. This effect has not been reported in humans.

Here's the gist: schisandra may block a liver enzyme called CYP2C19, and pentamidine is on the list of drugs that enzyme can help process. In theory, slowing that breakdown could let pentamidine build up a bit more in your body, which might mean a higher chance of its known side effects. With pentamidine that can include low blood sugar, low blood pressure, kidney strain, and heart rhythm changes (a longer QT interval), so it's not something to take lightly.

That said, this concern comes only from lab and animal work, never confirmed in people, so it stays theoretical. Pentamidine is also given as an injection in a medical setting where you're being watched. Still, please tell the team caring for you before adding schisandra so they can weigh it for your situation.

Because of its serious risks, Pentam 300 should only be given to patients in whom Pneumocystis carinii infection has actually been confirmed.
