Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

Here's the gist with Duavee (the estrogen plus bazedoxifene combo). Both of these hormones are cleared in part by a liver enzyme called CYP3A4. Schisandra tends to slow down that same enzyme, so in theory it could let more of the medication build up in your body. That might mean stronger estrogen-like effects, such as breast tenderness, bloating, headaches, or nausea, and at least on paper a bumped-up risk of clotting concerns that already come with these hormones.

The honest part: this is graded on lab and animal data, not solid human studies, so we don't know how big a deal it really is in everyday life. I wouldn't panic, but I'd run it by your pharmacist or doctor before pairing them rather than guessing.

Schisandra might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that schisandra extracts and constituents such as schisandrin B inhibit P-glycoprotein mediated efflux in intestinal cells and in P-glycoprotein over-expressing cell lines. Additionally, a small clinical study shows that schisandra increases the peak concentration and overall exposure to talinolol, a P-glycoprotein probe substrate. Theoretically, schisandra might inhibit the efflux of other P-glycoprotein substrates.

Here's the gist for Duavee (bazedoxifene plus conjugated estrogens). One way your body limits how much of certain medicines gets absorbed is a little pump in your gut wall called P-glycoprotein. Schisandra may slow that pump down, at least based on lab and animal studies plus one small human study using a different drug. If that happens, slightly more of the bazedoxifene part could end up in your bloodstream.

In plain terms, you might get a touch more estrogen-related effect than expected, things like breast tenderness, leg swelling, headaches, or nausea. This is mostly theoretical and not well proven, so I wouldn't panic, but I also wouldn't combine them on your own. Run it by your pharmacist or doctor first, especially since hormone therapy already carries its own risks.

What Is Estrogen and Bazedoxifene Used For?

  • Estrogen and bazedoxifene tablets are used to treat hot flashes (sudden feelings of warmth, especially in the face, neck, and chest) in women who are experiencing menopause (stage of life when menstrual periods become less frequent and stop and women may experience other symptoms and body changes). Estrogen and bazedoxifene tablets are also used to prevent osteoporosis (condition in which the bones become thin and weak and break easily) in women who have undergone menopause. Estrogen is in a class of medications called hormones and bazedoxifene is in a class of medications called estrogen agonist–antagonists. Estrogen works by replacing estrogen that is normally produced by the body. Bazedoxifene is used to block the action of estrogen on the lining of the uterus, decreasing the risk of overgrowth that may lead to cancer.
