Why this applies to Hydrocortisone

Hydrocortisone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Schisandra interacts with.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

Here's the simple version: your body uses a liver enzyme called CYP3A4 to break down and clear out oral hydrocortisone (like Cortef). Schisandra appears to slow that enzyme down, so hydrocortisone may stick around at higher levels than usual. Hydrocortisone is an active steroid (not a prodrug), so more of it in your system means a stronger effect.

In practice, that could mean more of the usual steroid side effects, things like fluid retention, higher blood sugar, trouble sleeping, mood changes, or stomach irritation. Keep in mind this concern comes mostly from lab and animal studies, so it's a reasonable caution, not a proven danger. Topical creams absorb very little and are unlikely to matter. Before combining them, check with your pharmacist or doctor.

Schisandra might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that schisandra extracts and constituents such as schisandrin B inhibit P-glycoprotein mediated efflux in intestinal cells and in P-glycoprotein over-expressing cell lines. Additionally, a small clinical study shows that schisandra increases the peak concentration and overall exposure to talinolol, a P-glycoprotein probe substrate. Theoretically, schisandra might inhibit the efflux of other P-glycoprotein substrates.

Here's the basic idea with Schisandra and oral hydrocortisone (like Cortef). Schisandra may block a 'pump' in your gut called P-glycoprotein, which normally helps push some drugs back out. When that pump slows down, a little more of the medication can get absorbed. Since hydrocortisone is a P-glycoprotein substrate and works directly (it is not a prodrug), the theory is that Schisandra could nudge its levels up a bit, which might mean slightly stronger steroid effects or more side effects like fluid retention, raised blood sugar, mood changes, or trouble sleeping.

I want to be honest about the evidence here, though. This is based mostly on lab and animal studies, plus one small human study using a different drug, so it is theoretical for hydrocortisone. Casual food amounts likely matter less than concentrated supplements. If you take hydrocortisone regularly, just run the Schisandra idea past your pharmacist or doctor first.

What Is Hydrocortisone Used For?

Brand names: Cortef, Hydrocortone, Carmol HC cream, Hydrocortisone Iodoquinol Cream, Vytone Cream, Massengil Medicated Towelettes, Dioderm, Efcortelan Cream, Mildison, Alphaderm, Calmurid HC, Efcortelan Ointment, Texacort, Alkindi Sprinkle

Hydrocortisone comes in many products, and each one is approved for different uses. Which one you get depends on the condition being treated.

Cortef tablets are used for adrenal insufficiency, congenital adrenal hyperplasia, rheumatic disorders (such as rheumatoid arthritis and gouty arthritis) and collagen diseases such as lupus. Hydrocortisone sodium succinate and Solu-Cortef injections are used when oral therapy is not feasible, for severe allergic states, skin diseases, and adrenal insufficiency. Alkindi (oral granules) is used as replacement therapy in children with adrenocortical insufficiency. Cortenema, Hydrocortisone Rectal Suspension and Cortifoam are used for ulcerative colitis or ulcerative proctitis. Topical hydrocortisone butyrate and valerate products, including Evesse lotion, are used for itchy, inflamed skin conditions and atopic dermatitis.
