CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Prednisone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Schisandra interacts with.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist: your liver uses an enzyme called CYP3A4 to help break down and clear prednisone. Schisandra tends to slow that enzyme down, so prednisone could linger a bit longer and build up in your system. In plain terms, that may push prednisone's effects (and side effects) up a notch.
What might that look like? Things like trouble sleeping, a jittery or wired feeling, higher blood sugar, fluid retention or puffiness, mood changes, or stomach upset. The good news is the evidence here is mostly lab and animal based, not strong human data, so this is more of a caution than a sure thing. Before combining them, just run it past your pharmacist or doctor so they can keep an eye on how you feel.

P-GLYCOPROTEIN SUBSTRATES

In vitro research shows that schisandra extracts and constituents such as schisandrin B inhibit P-glycoprotein mediated efflux in intestinal cells and in P-glycoprotein over-expressing cell lines. Additionally, a small clinical study shows that schisandra increases the peak concentration and overall exposure to talinolol, a P-glycoprotein probe substrate. Theoretically, schisandra might inhibit the efflux of other P-glycoprotein substrates.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for you. Prednisone is one of many medicines that your body moves around using a little cellular pump called P-glycoprotein. Schisandra may slow that pump down. In theory, that could let a bit more prednisone hang around in your system, which might nudge up its effects and side effects, things like trouble sleeping, mood swings, stomach upset, higher blood sugar, or fluid retention.
I want to be honest about the strength of this, though. The evidence here comes mostly from lab and animal work, plus one small human study using a different drug. So this is more of a theoretical heads-up than a proven problem. If you take prednisone regularly and want to add schisandra, just run it by your pharmacist or doctor first.
