Ashwagandha and Prednisone

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Ashwagandha interacts with whole categories of medication. Prednisone falls into 2 of those categories, so it can interact with Ashwagandha in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

IMMUNOSUPPRESSANTS

1 of 2

Why this applies to Prednisone

Prednisone falls under IMMUNOSUPPRESSANTS — the group Ashwagandha interacts with.

Theoretically, taking ashwagandha might decrease the effects of immunosuppressants.
Ashwagandha has demonstrated immunostimulant effects in humans. Animal research has shown that ashwagandha can attenuate the immunosuppression caused by cyclophosphamide.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the idea behind this pairing. One reason people take prednisone is to calm down an overactive immune system, whether that's for arthritis, lupus, severe allergies, or another inflammatory condition. Ashwagandha seems to do the opposite in some studies, gently nudging the immune system to be more active. So in theory, ashwagandha could work against prednisone and make it a little less effective.

I want to be honest that this concern comes mostly from lab and animal research, not strong human studies, so it's more theoretical than proven. Still, if you're relying on prednisone to keep a flare or your symptoms under control, it makes sense to be cautious. Please don't change either one on your own. Just check with your pharmacist or doctor first, especially if you're on long-term steroid therapy.

Minor

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Prednisone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: your body uses an enzyme called CYP3A4 to help process prednisone. Lab studies suggest ashwagandha may speed that enzyme up a bit, which in theory could clear prednisone from your system a little faster. If that happened, you might get slightly less benefit from your dose, meaning the inflammation, swelling, or autoimmune symptoms prednisone is treating may not be controlled quite as well.

That said, I don't want to overstate this. The concern comes only from test-tube and animal research, not from real people, and it's rated as a minor, possible interaction. For most folks taking ashwagandha short-term, it's unlikely to cause a noticeable problem. Still, since prednisone is often used for serious conditions, it's worth a quick chat with your pharmacist or doctor before combining them.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

From our pharmacist’s overview on the Prednisone page

“Prednisone (Deltasone) suppresses the immune system to control inflammation, and that mechanism shapes most of what can go wrong on this 208-ingredient list, which I read as one to approach carefully rather than casually. Four entries rate major.”
