Ashwagandha + Clomipramine
Clomipramine brand names: Anafranil, Anafranil SR.
Clomipramine falls into 2 drug categories that Ashwagandha can interact with; the most serious is rated minor. Theoretically, ashwagandha might decrease the levels and clinical effects of CYP1A2 substrates.

These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Ashwagandha interacts with whole categories of medication. Clomipramine falls into 2 of those categories, so it can interact with Ashwagandha in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Minor).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 2Why this applies to Clomipramine
Clomipramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ashwagandha interacts with.
In vitro research shows that ashwagandha extract induces CYP1A2 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea with these two together. Your liver uses an enzyme called CYP1A2 to help break down clomipramine (Anafranil). Some lab studies suggest ashwagandha may speed that enzyme up, which could mean your body clears the medication a little faster. In theory, that might leave less clomipramine in your system, so it may not work quite as well for your OCD symptoms.
I want to be honest about how soft this evidence is. It comes from test-tube research, not real people, and it's rated as a minor, possible interaction. Clomipramine is not a prodrug, so this is about a slightly weaker effect, not a buildup. Still, if you notice your symptoms creeping back, check in with your pharmacist or doctor before changing anything.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Clomipramine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea: your body uses a liver enzyme called CYP3A4 to help clear out clomipramine. Some lab studies hint that ashwagandha may speed up that enzyme. If that happened in real life, your body might break down clomipramine a little faster, which could lower its levels and make it work less well. For an OCD medication, that might mean your symptoms aren't as well controlled.
I want to be honest about the strength of this, though. It's only based on test-tube and animal research, the rating is minor, and clomipramine is actually handled by several enzymes, not just this one. So a big real-world effect isn't likely. Still, if you take clomipramine and want to try ashwagandha, run it by your pharmacist or doctor first.

What These Ratings Mean
Every interaction above is graded three ways — here’s the key to each label.
Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.
- LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Clomipramine page
“A tricyclic antidepressant used mostly for OCD, clomipramine (Anafranil) is unforgiving of shifts in its blood level, which is why its 236 supplement interactions deserve genuine wariness. Most of the list involves metabolism: herbs that speed up or slow down the liver enzymes handling clomipramine can drag its level too low or push it…”

Clomipramine Is Linked to Lower Levels of 3 Nutrients
Natural Medicines records Clomipramine as a medicine that can lower these. This is not an interaction with Ashwagandha — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
