Hydrochlorothiazide falls under ANTIHYPERTENSIVE DRUGS — the group Ashwagandha interacts with.

Theoretically, taking ashwagandha with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that ashwagandha might lower systolic and diastolic blood pressure. Theoretically, ashwagandha might have additive effects when used with antihypertensive drugs and increase the risk of hypotension.

Here's the gist: hydrochlorothiazide is a water pill that lowers your blood pressure by helping your kidneys flush out extra salt and fluid. Ashwagandha, in some early animal studies, seemed to nudge blood pressure down on its own. So the concern is simple addition. If both are pushing your numbers in the same direction, your pressure could dip a little lower than expected.

If that happened, you might feel lightheaded, dizzy, or woozy when you stand up too fast. I want to be honest, though: this is a theoretical concern based mostly on lab and animal work, not strong human studies. For most folks it's not dramatic. Still, if you take both, keep an eye on how you feel and check in with your pharmacist or doctor before changing anything.

What Is Hydrochlorothiazide Used For?

Brand names: Esidrix, Hydro, Microzide, HydroDiuril, Hydro-D, Oretic, Aquazide H, Aquazide, HCTZ

Hydrochlorothiazide treats high blood pressure (hypertension) in adults and children, either alone or combined with other blood pressure medicines. Lowering blood pressure reduces the risk of strokes and heart attacks.

It also treats edema (fluid buildup) linked to congestive heart failure, hepatic cirrhosis (liver scarring), and kidney conditions including nephrotic syndrome. Hydrochlorothiazide tablets are additionally indicated for edema caused by corticosteroid or estrogen therapy, and edema from acute glomerulonephritis or chronic renal failure. Inzirqo (the oral suspension) is specifically approved for both hypertension and edema in adult and pediatric patients.
