Dextromethorphan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ashwagandha interacts with.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.

Here's the short version for cough relief. Dextromethorphan is partly broken down by a liver enzyme called CYP3A4. Lab studies (in test tubes, not people) hint that ashwagandha may speed that enzyme up. If that happened in the body, your dextromethorphan could clear a little faster and work a bit less well, meaning your cough might not be quieted as much as usual.

Honestly, this is a very weak, theoretical concern rated as minor, and there's no good human evidence behind it. Dextromethorphan is also a fairly forgiving, short-term medicine. I wouldn't lose sleep over it, but if your cough syrup just doesn't seem to be doing its job, mention it to your pharmacist or doctor.

Dextromethorphan also falls under Serotonergic Drugs.

Some animal studies have reported that ashwagandha can enhance serotonergic transmission by altering certain serotonin (5-HT) receptors. However, there is no evidence to suggest that ashwagandha increases the risk of serotonin-related effects, and there have been no published case reports of serotonin syndrome when combined with other serotonergic drugs. Nevertheless, due to the lack of extensive studies on the matter and the fact that ashwagandha appears to affect serotonergic pathways, it would be prudent to exercise caution when combining it with drugs that affect serotonin. Here's the situation with these two together. Dextromethorphan, the cough suppressant in many over-the-counter products like Robitussin and Benylin, gently nudges serotonin activity in the brain. Ashwagandha may do something similar based on some animal studies. In theory, stacking two things that touch serotonin could add up.

That said, I want to keep this in perspective. The evidence here is weak, and there are no real-world reports of serotonin problems when ashwagandha is combined with serotonergic medicines. For most healthy adults using cough syrup short-term, this isn't something to lose sleep over. Just be aware of signs like agitation, fast heartbeat, sweating, shivering, or confusion, and stop and call your doctor if those show up. When in doubt, run it by your pharmacist first.

What Is Dextromethorphan Used For?

Brand names: Hold, Benylin Adult, Benylin Pediatric, Theraflu Thin Strips Long Acting Cough, Neo Citran Thin Strips Cough

Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation, such as the kind that comes with the common cold or breathing in irritants. It also helps reduce the urge to cough so you — or your child — can get to sleep.

Some products, including CVS Long Acting Cough and Premier Value Tussin Cough oral liquids, are also indicated to help loosen phlegm (mucus) and thin bronchial secretions to help drain the airways.
