Nortriptyline falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ashwagandha interacts with.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP1A2 substrates.

Here's the gist for nortriptyline and ashwagandha. Your body uses an enzyme called CYP1A2 to help clear nortriptyline. Lab studies hint that ashwagandha may speed this enzyme up a bit. If that happened in real life, your body could break down nortriptyline a little faster, which might lower its level and make it work less well. You could notice your depression symptoms not being as well controlled.

That said, this is based only on test-tube research, not on people, so it's very much a theoretical concern rated as minor. I wouldn't lose sleep over it. Still, before you add ashwagandha, give your pharmacist or doctor a quick heads-up so they can keep an eye on how you're feeling.

Nortriptyline also falls under Serotonergic Drugs.

Some animal studies have reported that ashwagandha can enhance serotonergic transmission by altering certain serotonin (5-HT) receptors. However, there is no evidence to suggest that ashwagandha increases the risk of serotonin-related effects, and there have been no published case reports of serotonin syndrome when combined with other serotonergic drugs. Nevertheless, due to the lack of extensive studies on the matter and the fact that ashwagandha appears to affect serotonergic pathways, it would be prudent to exercise caution when combining it with drugs that affect serotonin. Here's the short version: nortriptyline raises serotonin and other brain chemicals to lift your mood, and ashwagandha may gently nudge serotonin pathways too. On paper, stacking two things that touch serotonin could add up. In practice, this is a minor, theoretical concern. The signal comes mostly from animal studies, and there are no real-world case reports of problems when ashwagandha is taken alongside serotonin-affecting antidepressants like this one.

So I wouldn't lose sleep over it, but it's worth a quick mention to your prescriber. If you ever notice things like agitation, a racing heart, sweating, shivering, or feeling unusually restless, let your doctor know. Please don't start or stop either one on your own. A quick chat with your pharmacist or doctor first is the safe move.
