Clarithromycin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ashwagandha interacts with.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for taking ashwagandha alongside clarithromycin (Biaxin). Lab studies hint that ashwagandha may speed up an enzyme called CYP3A4, which is one of the pathways your body uses to break down clarithromycin. If that happened in real life, your body could clear the antibiotic a little faster, leaving slightly lower levels in your blood. The concern there isn't side effects, it's that a weaker antibiotic level might not knock out your infection as well.
The good news: this is only a theoretical, test-tube finding, rated minor, so it's far from proven in people. Still, since finishing an antibiotic course properly really matters, it's worth a quick chat with your pharmacist before pairing the two. An easy option is simply holding off on ashwagandha until your antibiotic course is done.

It is generally well tolerated for short-term use in healthy adults, but it should be avoided in pregnancy and used cautiously by people with thyroid or liver problems. What Is Clarithromycin Used For?
Brand names: Biaxin, Biaxin XL
Clarithromycin tablets and oral suspension treat a range of mild to moderate bacterial infections in adults and children, including community-acquired pneumonia, acute sinus infections, acute bacterial flare-ups of chronic bronchitis, strep throat and tonsillitis, uncomplicated skin infections, and middle ear infections in children. Tablets are also used in adults to eliminate H. pylori bacteria (in combination with other medicines) to reduce the risk of duodenal ulcer recurrence.
Clarithromycin tablets — but not the extended-release tablet — are also used to treat and prevent disseminated Mycobacterium avium complex (MAC) infections in people with advanced HIV. Clarithromycin extended-release tablets are approved only for acute bacterial exacerbation of chronic bronchitis, acute maxillary sinusitis, and community-acquired pneumonia in adults — not for the other conditions listed above.
