Why this applies to Methotrexate
Methotrexate falls under HEPATOTOXIC DRUGS — the group Ashwagandha interacts with.
Ashwagandha has been linked to cases of acute hepatitis, liver failure, hepatic encephalopathy, autoimmune hepatitis, the need for liver transplantation, and death due to liver failure.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to know about methotrexate: it's already one of the tougher drugs on the liver. Even at the low weekly doses used for rheumatoid arthritis or psoriasis, your doctor checks liver enzymes regularly to make sure it isn't causing trouble.
Ashwagandha has been tied, in a number of case reports, to liver problems of its own (everything from mild hepatitis to serious liver injury). So the worry here isn't a chemical reaction between the two. It's simply that you'd be stacking two things that can each stress the liver, which theoretically raises the combined risk. This is based on how each one behaves, not on direct studies. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please run ashwagandha by your pharmacist or doctor before adding it.

Ashwagandha is an Ayurvedic herb most often taken to help with stress, anxiety, and sleep, and some small studies suggest it may help, though the evidence is still limited. It is generally well tolerated for short-term use in healthy adults, but it should be avoided in pregnancy and used cautiously by people with thyroid or liver problems. What Is Methotrexate Used For?
Brand names: Rheumatrex, Otrexup, Rasuvo, Reditrex
Methotrexate treats a wide range of serious conditions. In cancer care, it is used for acute lymphoblastic leukemia, non-Hodgkin lymphoma, osteosarcoma, breast cancer, head and neck cancer, and gestational trophoblastic neoplasia (a rare tumor related to pregnancy). It is also used to prevent and treat meningeal leukemia (cancer affecting the lining of the brain and spinal cord).
Outside of cancer, methotrexate is prescribed for adults with rheumatoid arthritis (RA), children with polyarticular juvenile idiopathic arthritis (a form of childhood arthritis), and adults with severe psoriasis that hasn't responded well to other treatments.
