Hydrocortisone falls under IMMUNOSUPPRESSANTS — the group Ashwagandha interacts with.
Ashwagandha has demonstrated immunostimulant effects in humans. Animal research has shown that ashwagandha can attenuate the immunosuppression caused by cyclophosphamide.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
When you take hydrocortisone for something like arthritis, severe allergies, lupus, or another inflammatory condition, part of what it does is calm down an overactive immune system. Ashwagandha may pull in the opposite direction. It has shown immune-stimulating effects, so in theory it could chip away at how well hydrocortisone holds your immune response in check, meaning your symptoms might not stay as controlled.
I want to be honest about the strength of this concern: it's mostly based on lab and animal studies, not solid human trials, so it's more of a theoretical worry than a proven problem. Still, if your hydrocortisone is keeping a real condition in check, it's worth a quick chat with your pharmacist or doctor before adding ashwagandha. Don't change your dose on your own.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for these two together. Your liver uses an enzyme called CYP3A4 to clear hydrocortisone (Cortef) from your body. In lab and animal studies, ashwagandha seems to speed that enzyme up a bit. In theory, that could mean your body breaks down hydrocortisone a little faster, leaving slightly less of it working for you.
If you take hydrocortisone to replace a hormone your body isn't making, weaker effect could matter more, with things like fatigue, low energy, or feeling run down. But I want to be honest with you: this is theoretical, the evidence is weak (lab data only), and it's rated minor. I wouldn't lose sleep over it. Still, run it by your pharmacist or doctor before starting ashwagandha so they can keep an eye on how you feel.

Hydrocortisone is used to calm skin inflammation and relieve itching caused by conditions that respond to corticosteroid treatment. In adults, it treats a broad range of inflammatory skin conditions. In children as young as 3 months old, it is specifically approved for mild to moderate atopic dermatitis (eczema).
It is also used in rectal foam form to treat inflammation of the lower bowel, such as ulcerative proctitis.
