Esterified Estrogens falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ashwagandha interacts with.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.

Here's the basic idea with esterified estrogens (Menest, Estratab) and ashwagandha. Your liver uses an enzyme called CYP3A4 to help process estrogen. Lab studies suggest ashwagandha may speed up that enzyme, which could in theory lower your estrogen levels a bit. If that happened, your hormone therapy might not work quite as well, so you could notice symptoms creeping back, like hot flashes, night sweats, or vaginal dryness.

I want to be honest about the strength of this, though. It's only based on test-tube research, not real people, and it's rated as a minor, possible interaction. So this is more of a theoretical concern than something I'd expect to cause trouble. If you'd like to use ashwagandha, just run it by your pharmacist or doctor first.
