Ashwagandha and Prednisolone

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Ashwagandha interacts with whole categories of medication. Prednisolone falls into 2 of those categories, so it can interact with Ashwagandha in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

IMMUNOSUPPRESSANTS

1 of 2

Why this applies to Prednisolone

Prednisolone falls under IMMUNOSUPPRESSANTS — the group Ashwagandha interacts with.

Theoretically, taking ashwagandha might decrease the effects of immunosuppressants.
Ashwagandha has demonstrated immunostimulant effects in humans. Animal research has shown that ashwagandha can attenuate the immunosuppression caused by cyclophosphamide.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the idea behind this pairing. Prednisolone is often prescribed to calm down an overactive immune system, whether that's for arthritis, asthma, a flare-up, or to protect a transplanted organ. Ashwagandha, on the other hand, has shown some immune-boosting activity in lab and animal studies. So in theory, ashwagandha could nudge your immune system in the opposite direction from what prednisolone is trying to do, potentially making the medication work a little less well.

I want to be honest about the strength here: this is mostly theoretical and based on test-tube and animal data, not solid human studies, so it's a possible concern rather than a proven one. Still, if you're taking prednisolone for something serious like preventing transplant rejection or controlling an autoimmune condition, I'd play it safe and check with your pharmacist or doctor before adding ashwagandha.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Prednisolone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist of pairing ashwagandha with prednisolone. Your liver uses an enzyme called CYP3A4 to help process this steroid, and some early lab research hints that ashwagandha may speed that enzyme up a bit. If that happened in real life, your body could clear prednisolone a little faster, which in theory might make it work slightly less well. For something you're relying on, like controlling inflammation, a flare of arthritis, an allergic reaction, or transplant protection, that matters.

That said, this is a minor, theoretical concern based only on test-tube studies, not on people, so I wouldn't lose sleep over it. Just don't stop or change either one on your own. Mention the ashwagandha to your pharmacist or doctor so they can keep an eye on how well your prednisolone is working.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
