Ashwagandha and Topiramate

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CNS DEPRESSANTS

Why this applies to Topiramate

Topiramate falls under CNS DEPRESSANTS — the group Ashwagandha interacts with.

Theoretically, taking ashwagandha might increase the sedative effects of CNS depressants.
Ashwagandha seems to have sedative effects. Theoretically, this may potentiate the effects of barbiturates, other sedatives, and anxiolytics.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: ashwagandha can have a calming, drowsy effect of its own. Topiramate (Topamax and its other brands) already tends to slow people down a bit, often causing tiredness, foggy thinking, dizziness, or trouble concentrating. Stack the two together and those sleepy, sluggish feelings could add up and feel stronger.

I want to be honest about how solid this is. This concern is theoretical, based on how ashwagandha behaves in the body, not on real reports of problems with topiramate specifically. So it's worth knowing, not worth panicking over. If you take topiramate, just loop in your pharmacist or doctor before adding ashwagandha, and go easy with driving or anything risky until you see how the combo affects you.

About Topiramate

What Is Topiramate Used For?

Brand names: Topamax, Trokendi XR, Qudexy XR, Eprontia

  • Topiramate is used alone or with other medications to treat certain types of seizures including primary generalized tonic-clonic seizures (formerly known as a grand mal seizure; seizure that involves the entire body) and partial onset seizures (seizures that involve only one part of the brain). Topiramate is also used with other medications to control seizures in people who have Lennox-Gastaut syndrome (a disorder that causes seizures and developmental delays). Topiramate is also used to prevent migraine headaches but not to relieve the pain of migraine headaches when they occur. Topiramate is in a class of medications called anticonvulsants. It works by decreasing abnormal excitement in the brain.
