Selenium and Prednisone

Here is the interaction we found between these two, with the evidence behind it.

Moderate

IMMUNOSUPPRESSANTS

Why this applies to Prednisone

Prednisone falls under IMMUNOSUPPRESSANTS — the group Selenium interacts with.

Theoretically, selenium supplementation may reduce the effectiveness of immunosuppressant therapy.
In vitro research and preliminary clinical evidence suggests that selenium may stimulate the immune system.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here is the idea behind pairing selenium with prednisone. Prednisone is a steroid that calms down an overactive immune system, which is exactly why it helps with things like arthritis flares, lupus, asthma, and severe allergic reactions. Selenium, on the other hand, may gently nudge the immune system to be more active. So in theory, taking a lot of selenium could work against what prednisone is trying to do.

I want to be honest with you though: this concern comes from lab and test-tube studies, not from solid human research, so it is more theoretical than proven. Normal dietary amounts of selenium are very unlikely to matter. If you are on high-dose selenium supplements, just mention it to your doctor or pharmacist so they can keep an eye on whether your prednisone is still controlling your symptoms.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.

From our pharmacist’s overview on the Prednisone page

“Prednisone (Deltasone) suppresses the immune system to control inflammation, and that mechanism shapes most of what can go wrong on this 208-ingredient list, which I read as one to approach carefully rather than casually. Four entries rate major.”

About Selenium

What is Selenium?

  • Thyroid health support
  • Antioxidant support
  • Immune support
  • General nutrient supplementation
  • Reproductive/fertility support

Selenium is an essential trace mineral your body needs in small amounts for thyroid function, antioxidant defense, and immune health. Most people who eat a varied diet get enough, and supplements are mainly useful for those who are deficient. Because too much selenium can be toxic, stick to recommended amounts and check with your pharmacist or doctor before supplementing.
