Why this applies to Hydrocortisone
Hydrocortisone falls under IMMUNOSUPPRESSANTS — the group Selenium interacts with.
In vitro research and preliminary clinical evidence suggests that selenium may stimulate the immune system.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
When you take hydrocortisone, part of what it does is calm down an overactive immune system. That is why it helps with things like severe allergic reactions, lupus, and certain inflammatory conditions. The concern here is that selenium may gently nudge the immune system in the opposite direction. In theory, that could work against hydrocortisone's calming effect and make it a touch less effective.
I want to be honest about how soft this evidence is. It comes mostly from lab and early studies, not solid human trials, so this is more of a theoretical worry than something proven. Normal dietary selenium is very unlikely to matter. If you take a selenium supplement, just mention it to your pharmacist or doctor so they can keep an eye on how well your hydrocortisone is working.

Hydrocortisone comes in many products, and each one is approved for different uses. Which one you get depends on the condition being treated.
Cortef tablets are used for adrenal insufficiency, congenital adrenal hyperplasia, rheumatic disorders (such as rheumatoid arthritis and gouty arthritis) and collagen diseases such as lupus. Hydrocortisone sodium succinate and Solu-Cortef injections are used when oral therapy is not feasible, for severe allergic states, skin diseases, and adrenal insufficiency. Alkindi (oral granules) is used as replacement therapy in children with adrenocortical insufficiency. Cortenema, Hydrocortisone Rectal Suspension and Cortifoam are used for ulcerative colitis or ulcerative proctitis. Topical hydrocortisone butyrate and valerate products, including Evesse lotion, are used for itchy, inflamed skin conditions and atopic dermatitis.
