Sodium and Prednisone

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CORTICOSTEROIDS

Why this applies to Prednisone

Prednisone falls under CORTICOSTEROIDS — the group Sodium interacts with.

Concomitant use of mineralocorticoids and some glucocorticoids with sodium supplements might increase the risk of hypernatremia.
Mineralocorticoids and some glucocorticoids (corticosteroids) cause sodium retention. This effect is dose-related and depends on mineralocorticoid potency. It is most common with hydrocortisone, cortisone, and fludrocortisone, followed by prednisone and prednisolone.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version. Prednisone can make your body hold onto sodium and water. If you're also taking sodium on top of that, the two can stack up and push your sodium and fluid levels higher than they should be. In real life that might show up as puffiness or swelling in the ankles, weight gain from fluid, or higher blood pressure. This matters more if you have heart, kidney, or blood pressure problems.

Now, the good news: this is a theoretical concern based on how these drugs work, and prednisone causes much less sodium retention than stronger steroids. Ordinary salt in food usually isn't the issue. It's deliberate sodium supplements that I'd ask about. Before adding any, please check with your doctor or pharmacist first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Prednisone page

“Prednisone (Deltasone) suppresses the immune system to control inflammation, and that mechanism shapes most of what can go wrong on this 208-ingredient list, which I read as one to approach carefully rather than casually. Four entries rate major.”

About Sodium

What is Sodium?

  • Replacing fluids and electrolytes
  • Preventing dehydration during exercise or illness
  • Treating low blood sodium (under medical care)
  • Seasoning and food preservation
  • Antacid (as sodium bicarbonate)

Sodium is an essential mineral and electrolyte your body needs to balance fluids, support nerves, and help muscles work. Most people in modern diets get more than enough—often too much—from food, so supplements are rarely needed except for specific medical reasons like dehydration or low blood sodium. Talk to your doctor or pharmacist before deliberately adding sodium, especially if you have high blood pressure, heart, or kidney problems.
