Sodium and Sodium Phenylbutyrate

Here is the interaction we found between these two, with the evidence behind it.

Moderate

SODIUM-CONTAINING DRUGS

Why this applies to Sodium Phenylbutyrate

Sodium Phenylbutyrate falls under SODIUM-CONTAINING DRUGS — the group Sodium interacts with.

Concomitant use of sodium-containing drugs with additional sodium from dietary or supplemental sources may increase the risk of hypernatremia and long-term sodium-related complications.
The Chronic Disease Risk Reduction (CDRR) intake level of 2.3 grams of sodium daily indicates the intake at which it is believed that chronic disease risk increases for the apparently healthy population. Some medications contain high quantities of sodium. When used in conjunction with sodium supplements or high-sodium diets, the CDRR may be exceeded. Additionally, concomitant use may increase the risk for hypernatremia; this risk is highest in the elderly and people with other risk factors for electrolyte disturbances.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing about sodium phenylbutyrate: the medicine itself already carries a hefty load of sodium right in the molecule. People usually take it for urea cycle disorders, and the daily doses can add up to a surprising amount of sodium on their own. So if you stack extra sodium on top, whether from a supplement or a very salty diet, you can push your total intake higher than is healthy.

This is a theoretical concern based on the chemistry, not a dramatic reaction, but it's a real one. Too much sodium can mean fluid buildup, swelling, higher blood pressure, and in some cases a rise in blood sodium (hypernatremia), especially in older folks or anyone with heart or kidney issues. Please don't change anything on your own. Just ask your doctor or pharmacist before adding any sodium supplement.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Sodium Phenylbutyrate page

“Sodium phenylbutyrate (Buphenyl, Pheburane, Olpruva) keeps ammonia in check in people with urea cycle disorders, so a supplement that blunts its absorption is not a small problem, it is the difference between a controlled ammonia level and a dangerous one. Seventeen of the 19 ingredients on this page, mostly fibers, gums, and mucilages…”

About Sodium

What is Sodium?

  • Replacing fluids and electrolytes
  • Preventing dehydration during exercise or illness
  • Treating low blood sodium (under medical care)
  • Seasoning and food preservation
  • Antacid (as sodium bicarbonate)

Sodium is an essential mineral and electrolyte your body needs to balance fluids, support nerves, and help muscles work. Most people in modern diets get more than enough—often too much—from food, so supplements are rarely needed except for specific medical reasons like dehydration or low blood sodium. Talk to your doctor or pharmacist before deliberately adding sodium, especially if you have high blood pressure, heart, or kidney problems.
