Why this applies to Brexpiprazole
Brexpiprazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Schisandra interacts with.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to know about Rexulti (brexpiprazole): your liver relies heavily on an enzyme called CYP3A4 to break it down and clear it out. Schisandra tends to slow that same enzyme down. So when you pair the two, your body may clear brexpiprazole more slowly, and the drug can build up a bit higher than expected.
In real terms, that could mean more of brexpiprazole's usual side effects, like drowsiness, restlessness or feeling jittery, dizziness, or weight changes. Brexpiprazole is not a prodrug, so more of it in your system means a stronger effect, not a weaker one. This concern comes mostly from lab and animal data, so it's not proven in people. Still, before adding schisandra, please check with your pharmacist or doctor first.

What Is Brexpiprazole Used For?
Brand name: Rexulti
Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.
Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
