Why this applies to Formoterol

Formoterol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Schisandra interacts with.

Theoretically, schisandra might increase the levels and clinical effects of CYP2C19 substrates.
In vitro research shows that schisandra inhibits CYP2C19, and animal research shows that schisandra increases the concentration of voriconazole, a CYP2C19 substrate. Theoretically, schisandra may also inhibit the metabolism of other CYP2C19 substrates. This effect has not been reported in humans.

Here's the gist for your inhaler. Schisandra has been shown in lab and animal studies to slow down a liver enzyme called CYP2C19. The idea is that if your body breaks a drug down more slowly, that drug can build up a bit and its effects get stronger. Formoterol is not a prodrug, so in theory more of it staying around could mean more of its usual effects, like a faster or pounding heartbeat, shakiness or tremor, nervousness, or a headache.

That said, I want to keep this in perspective. This was only seen in test tubes and animals, never reported in real people, and formoterol isn't heavily dependent on this one enzyme. The concern is largely theoretical. Still, if you notice jitteriness or heart racing, or before adding schisandra, check with your pharmacist or doctor.

Here's the gist with Schisandra and Formoterol. In lab and animal studies, schisandra seems to speed up a liver enzyme called CYP2C9, which can make some medicines clear out of the body a bit faster. If that happened with formoterol, your inhaler could in theory work a little less well, so you might notice more wheezing, shortness of breath, or chest tightness creeping back.

The big caveat: this is only a theoretical concern. It has never actually been seen in people, and formoterol isn't a prodrug that needs activating, so the worry is fairly low. Still, since this controls your breathing, mention any schisandra supplement to your pharmacist or doctor first and keep an eye on how well your inhaler is working.

What Is Formoterol Oral Inhalation Used For?

Brand names: Foradil Aerolizer, Atimos Modulite, Oxis Turbohaler, Perforomist, Foradil Certihaler

Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.

Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
