Why this applies to Apixaban

Apixaban falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Schisandra interacts with.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

Here's the short version for Eliquis (apixaban): your body partly clears this blood thinner using a liver enzyme called CYP3A4. Schisandra appears to slow that enzyme down. When the enzyme works slower, apixaban can hang around longer and build up in your system, which means a stronger blood-thinning effect than intended.

In real terms, that could nudge up your bleeding risk, things like easy bruising, nosebleeds, bleeding gums, or in rarer cases more serious bleeding. The good news is that the evidence here mostly comes from lab and animal studies, not solid human data, so this is more of a caution than a proven problem. Still, because apixaban is a drug where small changes matter, please check with your pharmacist or doctor before pairing it with schisandra.

Schisandra might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that schisandra extracts and constituents such as schisandrin B inhibit P-glycoprotein mediated efflux in intestinal cells and in P-glycoprotein over-expressing cell lines. Additionally, a small clinical study shows that schisandra increases the peak concentration and overall exposure to talinolol, a P-glycoprotein probe substrate. Theoretically, schisandra might inhibit the efflux of other P-glycoprotein substrates.

Here's the gist for Eliquis (apixaban): this blood thinner is partly carried out of your body by a transporter pump called P-glycoprotein. Schisandra appears to slow that pump down. If the pump works slower, more apixaban can stay in your system, which could nudge its blood-thinning effect a bit higher.

In real life that means a slightly raised chance of bleeding problems, like easy bruising, nosebleeds, bleeding gums, or in rare cases more serious bleeding. I'll be straight with you though: this concern comes mostly from lab and animal studies, not from people taking apixaban, so it's more of a theoretical caution than a proven danger. Still, since apixaban is a drug where small changes matter, please check with your pharmacist or doctor before adding schisandra.

About Apixaban

What Is Apixaban Used For?

Brand name: Eliquis

Apixaban is used to reduce the risk of stroke and dangerous clots traveling through the body in adults with nonvalvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). It is also used to prevent deep vein thrombosis (DVT — a blood clot in the leg) in adults after hip or knee replacement surgery, and to treat DVT and pulmonary embolism (PE — a blood clot in the lungs) in adults, including reducing the risk of those clots coming back.

Eliquis (the brand-name tablet and kit) is additionally approved to treat venous thromboembolism (VTE — blood clots in the veins) and reduce the risk of recurrent VTE in pediatric patients from birth and older, after at least 5 days of initial anticoagulant treatment. The generic apixaban tablet carries the adult indications only.
