Why this applies to Apixaban

Apixaban falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Gymnema interacts with.

Theoretically, gymnema might increase levels of drugs metabolized by CYP3A4.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Here's the gist for Eliquis (apixaban): your liver partly clears this blood thinner using an enzyme called CYP3A4. The thinking is that gymnema might slow that enzyme a little, which could let apixaban build up. Since apixaban works as-is (it isn't a prodrug that needs activating), higher levels would mean a stronger blood-thinning effect and a greater chance of bruising or bleeding, like nosebleeds, bleeding gums, or blood in your urine or stool.

That said, this concern is mostly theoretical. It comes from one rat-tissue study, while human-tissue and animal research showed no real effect on CYP3A4. A true problem is unlikely. Still, because bleeding is serious, please run gymnema by your pharmacist or doctor before combining the two.

About Apixaban

What Is Apixaban Used For?

Brand name: Eliquis

Apixaban is used to reduce the risk of stroke and dangerous clots traveling through the body in adults with nonvalvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). It is also used to prevent deep vein thrombosis (DVT — a blood clot in the leg) in adults after hip or knee replacement surgery, and to treat DVT and pulmonary embolism (PE — a blood clot in the lungs) in adults, including reducing the risk of those clots coming back.

Eliquis (the brand-name tablet and kit) is additionally approved to treat venous thromboembolism (VTE — blood clots in the veins) and reduce the risk of recurrent VTE in pediatric patients from birth and older, after at least 5 days of initial anticoagulant treatment. The generic apixaban tablet carries the adult indications only.
