Why this applies to Dronabinol

Dronabinol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Gymnema interacts with.

Theoretically, gymnema might increase or decrease levels of drugs metabolized by CYP2C9.
Animal research shows that gymnema can induce the CYP2C9 enzyme. In one animal study, gymnema caused a 2.4-fold increase in the clearance of tolbutamide, a CYP2C9 substrate, in rats. In vitro research also shows that gymnema can inhibit CYP2C9.

Here's the short version on pairing gymnema with dronabinol. Your liver uses an enzyme called CYP2C9 to break dronabinol down, and gymnema may nudge that enzyme in either direction. In lab and animal studies it has both sped up and slowed down CYP2C9, so we honestly can't predict the result in a real person.

If gymnema speeds the enzyme up, dronabinol could clear faster and work less well for nausea or appetite. If it slows the enzyme, dronabinol could build up and cause more drowsiness, dizziness, or a stronger "high" feeling. Keep in mind this is theoretical, based only on test-tube and animal data, so don't panic. Just mention to your pharmacist or doctor that you're using gymnema before combining the two.

Dronabinol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, gymnema might increase levels of drugs metabolized by CYP3A4.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Here's the gist of it. Your body relies on a liver enzyme called CYP3A4 to break down dronabinol. The thought is that gymnema might slow that enzyme down a little, which could let dronabinol build up slightly. If that happened, you'd most likely notice more of its usual effects, like drowsiness, dizziness, a foggy or 'high' feeling, or feeling lightheaded when you stand up.

That said, I wouldn't lose sleep over this one. The concern comes only from a single test-tube study using rat tissue. Other lab and animal research found no real effect on this enzyme, so any interaction is theoretical and unlikely to matter for most people. Still, since dronabinol already makes some folks woozy, it's worth a quick word with your pharmacist or doctor before adding gymnema.

What Is Dronabinol Used For?

Brand names: Marinol, Syndros

Dronabinol — available as capsules (Marinol, Dronabinol) and as an oral solution (Syndros, Dronabinol oral solution) — is approved for two conditions in adults. First, it treats anorexia (severely reduced appetite) associated with weight loss in patients with AIDS. Second, it controls nausea and vomiting caused by cancer chemotherapy in patients who have not gotten enough relief from standard anti-nausea treatments.

Both uses apply to all available formulations, but dosing timing and meal instructions differ between the capsule and oral solution products. Always follow the specific instructions for the product you have been prescribed.
