Gymnema and Testosterone

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Testosterone

Testosterone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Gymnema interacts with.

Theoretically, gymnema might increase levels of drugs metabolized by CYP3A4.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: your body uses an enzyme called CYP3A4 to help break down testosterone, and there's a thought that gymnema might slow that enzyme down a bit. If that happened, testosterone could clear a little slower and build up, which in theory might nudge up its effects or side effects (think things like acne, oily skin, mood shifts, or fluid retention).

But honestly, I wouldn't lose sleep over this one. The only hint of an issue comes from a single test-tube study in rat liver. Other lab and animal research found gymnema didn't affect CYP3A4 at all. So this is a very weak, theoretical concern. Still, since you're on testosterone and may be watching your blood sugar too, it's worth a quick word with your pharmacist or doctor before adding gymnema.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

These are real questions from readers, answered by the pharmacists at HelloPharmacist — written by a person, not generated from a database. Each one covers Gymnema or Testosterone, not necessarily both, but if one is close to what you were about to ask it will usually go further than a database entry can:

From our pharmacist’s overview on the Testosterone page

“The supplement list for testosterone therapy (Androgel, Testim, Nebido, and others) deserves real attention, mainly because five of its 170 entries are rated major and four of those are everyday items: grapefruit, St. John's wort, and alcohol in the form of beer or wine.”

About Gymnema

What is Gymnema?

Gymnema
  • Blood sugar support
  • Type 2 diabetes
  • Sugar cravings
  • Weight management
  • Cholesterol support

Gymnema is an Ayurvedic herb best known for possibly helping lower blood sugar and reducing the taste of sweetness on the tongue. Some early human studies are encouraging for blood sugar support, but the evidence is still limited and not strong enough to replace standard treatment. If you have diabetes or take other medicines, talk with your pharmacist or doctor before using it.

About Testosterone

What Is Testosterone Used For?

Brand names: Intrinsa, Testim, Androgel, Andropatch, Testogel, Nebido, Delatestryl, Natesto, Vogelxo

Testosterone replacement therapy is used in adult males whose bodies do not produce enough of their own testosterone. This includes primary hypogonadism — where the testes themselves fail due to causes such as Klinefelter's syndrome, injury, chemotherapy, or surgical removal — and hypogonadotropic hypogonadism, where the brain's signaling system (the hypothalamus or pituitary gland) fails to trigger testosterone production.

Testopel (subcutaneous pellets) may also be used to stimulate puberty in carefully selected males with clearly delayed puberty. The safety and efficacy of testosterone for age-related (late-onset) hypogonadism have not been established for most products, including AndroGel, Testim, Jatenzo, Kyzatrex, Natesto, Depo-Testosterone, Testosterone Cypionate, and Xyosted.
