Why this applies to Acalabrutinib

Acalabrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Gymnema interacts with.

Theoretically, gymnema might increase levels of drugs metabolized by CYP3A4.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Here's the short version: your body clears acalabrutinib using a liver enzyme called CYP3A4. The theory is that gymnema might slow that enzyme down a little, which could let acalabrutinib build up. If that happened, you might notice more of its usual side effects, like bruising or bleeding more easily, headaches, tiredness, or a higher risk of infection.

That said, I wouldn't lose sleep over this one. The concern comes only from a single test-tube study using rat liver, while studies in human liver tissue and live animals showed gymnema did not meaningfully change CYP3A4. Acalabrutinib is an active drug, not a prodrug, so the direction holds. Since this is a serious cancer medicine, please run gymnema past your oncologist or pharmacist first.

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
