Clozapine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Gymnema interacts with.

Theoretically, gymnema might increase levels of drugs metabolized by CYP1A2.
Animal and in vitro research shows that gymnema can inhibit the CYP1A2 enzyme. In one animal study, oral administration of gymnema for 7 days increased the plasma concentrations of phenacetin, a CYP1A2 substrate, by about 1.4-fold and reduced the clearance of phenacetin by about 29%.

Here's the idea behind this pairing. Your body relies on a liver enzyme called CYP1A2 to clear clozapine out of your system. Lab and animal studies suggest gymnema may slow that same enzyme down a bit. If that happened in people, clozapine could clear more slowly and build up, which could mean stronger effects and more side effects like heavy sedation, dizziness, drooling, constipation, a racing heart, or a drop in blood pressure.

That said, this is theoretical and only based on lab and animal data, so we don't really know if it matters in real life. Because clozapine needs close monitoring anyway, please loop in your psychiatrist or pharmacist before adding gymnema rather than starting it on your own.

What Is Clozapine Used For?

Brand names: Clozaril, Versacloz

Clozapine is used for two specific situations. First, it is prescribed for people with severe schizophrenia who have already tried other antipsychotic medications and have not gotten adequate relief. It is reserved for this group because of its serious risks.

Second, clozapine is used to lower the chances of repeated suicidal behavior in people with schizophrenia or schizoaffective disorder who are considered to be at ongoing, chronic risk. It is not a first-choice medication — it is used when the need is serious and other options have fallen short.
