Imipramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Gymnema interacts with.
Animal and in vitro research shows that gymnema can inhibit the CYP1A2 enzyme. In one animal study, oral administration of gymnema for 7 days increased the plasma concentrations of phenacetin, a CYP1A2 substrate, by about 1.4-fold and reduced the clearance of phenacetin by about 29%.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea with imipramine and gymnema. Your liver uses an enzyme called CYP1A2 to help break down imipramine. Lab and animal studies hint that gymnema may slow that enzyme down a bit. If that happens in people, imipramine could clear out a little more slowly and build up, which might mean stronger effects and more side effects like drowsiness, dry mouth, constipation, blurred vision, dizziness, or a faster heartbeat.
I'll be honest with you: this is a theoretical concern based only on lab and animal data, not real human studies, so I wouldn't lose sleep over it. Still, imipramine is a medication where dosing matters, so it's worth a quick chat with your pharmacist or doctor before adding gymnema.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Animal research shows that gymnema can induce the CYP2C9 enzyme. In one animal study, gymnema caused a 2.4-fold increase in the clearance of tolbutamide, a CYP2C9 substrate, in rats. In vitro research also shows that gymnema can inhibit CYP2C9.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture: imipramine is partly handled by a liver enzyme called CYP2C9, and gymnema may nudge that enzyme one way or the other. The catch is that the evidence comes only from lab and animal studies, so this is theoretical for people. In animals, gymnema sped the enzyme up (which could lower imipramine levels and make it work less well), but in test-tube studies it slowed the enzyme down (which could raise levels and bring on more side effects like drowsiness, dry mouth, constipation, or a racing heart).
Because we can't say which direction wins in real life, I wouldn't panic, but I would loop in your pharmacist or doctor before adding gymnema. They can watch your mood and any side effects and keep you safe.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Here's the simple version: your body uses an enzyme called CYP3A4 to help break down imipramine. The idea on file is that gymnema might slow that enzyme a little, which could let imipramine build up slightly. If that happened, you might notice more of this drug's usual side effects, things like dry mouth, constipation, drowsiness, dizziness on standing, or a faster heartbeat.
But honestly, I wouldn't lose sleep over this one. The concern comes only from a single test-tube study in rat liver. Studies using human tissue and live animals showed no real effect, so a meaningful interaction is unlikely. Still, since gymnema can also lower blood sugar, it's worth a quick chat with your pharmacist or doctor before adding it.

What Is Imipramine Used For?
Brand names: Tofranil, Tofranil PM
Imipramine is used to relieve symptoms of depression. Both Imipramine Hydrochloride tablets and Imipramine Pamoate capsules are approved for this use. Endogenous depression — the kind that seems to arise from within rather than a clear outside cause — tends to respond especially well. It may take one to three weeks before you notice the full benefit.
Imipramine Hydrochloride tablets are also approved as a short-term, add-on treatment to help reduce nighttime bedwetting (nocturnal enuresis) in children aged 6 and older, after a doctor has ruled out physical causes.
