Amitriptyline, Chlordiazepoxide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Gymnema interacts with.
Animal and in vitro research shows that gymnema can inhibit the CYP1A2 enzyme. In one animal study, oral administration of gymnema for 7 days increased the plasma concentrations of phenacetin, a CYP1A2 substrate, by about 1.4-fold and reduced the clearance of phenacetin by about 29%.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Limbitrol DS, which combines amitriptyline (an older antidepressant) with chlordiazepoxide (a sedative similar to Valium). The amitriptyline part is partly handled by a liver enzyme called CYP1A2. In lab and animal studies, gymnema seems to slow that enzyme down a bit. If that played out in people, amitriptyline might clear a little slower and build up, which could mean more drowsiness, dry mouth, constipation, blurred vision, dizziness, or a faster heartbeat. Combined with chlordiazepoxide's own sedation, the sleepiness could feel stronger.
That said, this is only a theoretical concern based on weak animal and test-tube data, not human studies. Still, since this is a sensitive combo, it's worth a quick chat with your pharmacist or doctor before adding gymnema.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Animal research shows that gymnema can induce the CYP2C9 enzyme. In one animal study, gymnema caused a 2.4-fold increase in the clearance of tolbutamide, a CYP2C9 substrate, in rats. In vitro research also shows that gymnema can inhibit CYP2C9.

Here's the honest picture with Limbitrol DS (a combo of amitriptyline and chlordiazepoxide) and Gymnema. The concern on file is that Gymnema may bump the activity of a liver enzyme called CYP2C9, which helps process some medicines. But this evidence comes only from lab dishes and animal studies, and the results actually point in two directions: some show Gymnema speeding the enzyme up, others show it slowing it down. So we honestly can't say which way this would go in a person.
In theory, if it sped clearance up, your medicine might feel less effective. If it slowed clearance, you could see more drowsiness, dizziness, dry mouth, or grogginess. This is unproven, so don't make changes on your own. Just check with your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Limbitrol DS combines amitriptyline (an antidepressant) with chlordiazepoxide (a sedative, anti-anxiety medicine). The body partly clears both of these using a liver enzyme called CYP3A4. The thinking here is that gymnema might slow that enzyme down a little, which could let these drugs build up slightly. If that happened, you might notice more of their usual effects, like drowsiness, dizziness, dry mouth, constipation, or feeling foggy.
Honestly though, the evidence behind this is very weak. It comes only from a lab study using rat tissue, while human-tissue and animal studies showed no real effect on CYP3A4. So a meaningful interaction is unlikely. Since gymnema can also lower blood sugar on its own, it's still smart to check with your pharmacist or doctor before combining them.
