Apixaban falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Hemp interacts with.

Theoretically, hemp seed might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In animal research, hemp seed at 5% of the diet inhibits platelet aggregation in vitro. However, in human research, taking hemp seed oil 2 grams daily for 12 weeks does not inhibit the aggregation of platelets in vitro.

Apixaban (Eliquis) is a blood thinner that lowers your blood's ability to clot, so anything that nudges that effect further deserves a quick look. The concern here is mostly theoretical. In a lab and animal setting, hemp seed seemed to make platelets a little less sticky, which could in theory add to apixaban's blood-thinning effect and slightly raise bleeding risk.

But here's the reassuring part: in actual human testing, hemp seed oil at 2 grams a day for 12 weeks did not change platelet behavior. So eating hemp seeds or using hemp oil in normal food amounts is very unlikely to cause trouble. Just watch for easy bruising, nosebleeds, or bleeding gums, and check with your pharmacist before using concentrated hemp supplements.

Apixaban also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, hemp might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that hemp induces CYP3A4 enzymes.

Here's the short version: your body partly clears Eliquis (apixaban) using an enzyme called CYP3A4. Some early lab research suggests hemp may speed that enzyme up a little. If that happened in real life, your body could break down apixaban a bit faster, which in theory could leave slightly less of it in your blood. Since apixaban is a blood thinner that prevents clots and strokes, weaker levels could mean a touch less protection.

I don't want to overstate this. It's rated minor and comes only from test-tube studies, not from people, so we really don't know if normal hemp food or oil matters at all. Apixaban also isn't a prodrug, so the direction here is reduced effect, not more bleeding. Before adding concentrated hemp supplements, just check with your pharmacist or doctor.

About Apixaban

What Is Apixaban Used For?

Brand name: Eliquis

Apixaban is used to reduce the risk of stroke and dangerous clots traveling through the body in adults with nonvalvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). It is also used to prevent deep vein thrombosis (DVT — a blood clot in the leg) in adults after hip or knee replacement surgery, and to treat DVT and pulmonary embolism (PE — a blood clot in the lungs) in adults, including reducing the risk of those clots coming back.

Eliquis (the brand-name tablet and kit) is additionally approved to treat venous thromboembolism (VTE — blood clots in the veins) and reduce the risk of recurrent VTE in pediatric patients from birth and older, after at least 5 days of initial anticoagulant treatment. The generic apixaban tablet carries the adult indications only.
