Hemp and Verapamil

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Hemp interacts with whole categories of medication. Verapamil falls into 3 of those categories, so it can interact with Hemp in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Minor).

Minor

ANTIHYPERTENSIVE DRUGS

1 of 3

Why this applies to Verapamil

Verapamil falls under ANTIHYPERTENSIVE DRUGS — the group Hemp interacts with.

Theoretically, hemp seed protein may have additive effects with antihypertensive drugs.
In a hypertensive animal model, hemp seed protein hydrolysate reduced systolic blood pressure by a mechanism possibly involving the inhibition of renin and angiotensin converting enzyme (ACE) activities. However, there was no effect of hemp seed protein on blood pressure in normotensive animals. Furthermore, hempseed oil consumption does not seem to reduce blood pressure in humans.
Severity Minor Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: there's a theory that hemp seed protein might gently lower blood pressure on its own. If that were true, stacking it with verapamil (which already relaxes your blood vessels and slows your heart rate) could in theory add up, leaving you a little dizzy, lightheaded, or tired, especially when you stand up quickly.

But I want to be honest about how thin this evidence is. The blood-pressure effect was only seen in animals bred to be hypertensive, and human studies of hempseed oil showed no drop in blood pressure. So for most people eating hemp seeds as food, this isn't something to worry about. If you use concentrated hemp protein supplements and take verapamil, just mention it to your pharmacist or doctor so they can keep an eye on your readings.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

2 of 3

Verapamil also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, hemp might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that hemp induces CYP1A2 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for hemp and verapamil. In lab studies (not in people yet), hemp appears to speed up an enzyme called CYP1A2, which is one of the pathways your body uses to process certain medicines. Since verapamil is the active drug itself (not a prodrug), faster clearance could in theory lower its levels a bit, meaning slightly weaker blood pressure control, less help with chest pain, or a heart rate that isn't as well managed.

That said, this is a minor, theoretical concern based only on test-tube data, so I wouldn't lose sleep over it. Eating hemp seeds or hemp oil as food is very unlikely to matter. If you're using concentrated hemp supplements, just mention it to your pharmacist or doctor so they can keep an eye on how you're doing.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the situation with hemp and verapamil. Your body uses an enzyme called CYP3A4 to clear verapamil out of your system. Some early lab work suggests hemp might speed that enzyme up a bit. If that happened in real life, your body could break down verapamil a little faster, which in theory might make it work less well. That could mean your blood pressure creeping up, less control over chest pain, or a faster heart rate if you take it for rhythm problems.

The important part: this is only test-tube data, not something proven in people. Everyday hemp seeds or hemp oil in food are very unlikely to cause a problem. If you're thinking about concentrated hemp supplements, just run it by your pharmacist or doctor first so they can keep an eye on your numbers.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

From our pharmacist’s overview on the Verapamil page

“This is one of the longer interaction lists we publish, and with verapamil (Calan, Verelan) it warrants genuine care, starting with the ten major-rated entries. Verapamil lowers blood pressure and slows the heart, so most of the 295 ingredients on file either push that effect further than intended or change how much of the drug reaches…”

About Hemp

What is Hemp?

Hemp
  • Plant-based protein source
  • Heart health support
  • Skin and hair health
  • Reducing inflammation
  • General nutrition (omega fatty acids)

Hemp seeds and hemp seed oil are nutritious foods rich in protein, fiber, and healthy omega-3 and omega-6 fatty acids, and they are generally safe for most people as part of the diet. While the nutrition is real, strong human evidence that hemp supplements treat specific medical conditions is limited. Talk with your pharmacist or doctor before using hemp products for a health concern, especially if you are pregnant or breastfeeding.

About Verapamil

What Is Verapamil Used For?

Brand names: Isoptin SR, Verelan, Isoptin, Calan SR, Verelan SR, Calan, Covera HS, Verap, Veramil

Verapamil tablets and film-coated tablets are used to treat angina — including vasospastic (Prinzmetal's) angina, unstable angina, and chronic stable angina — as well as certain abnormal heart rhythms (arrhythmias) and high blood pressure (essential hypertension). For arrhythmias, regular-release tablets are used alongside digoxin to control heart rate in atrial flutter or atrial fibrillation, and to help prevent episodes of paroxysmal supraventricular tachycardia (SVT), a type of rapid heartbeat.

Extended-release and sustained-release oral forms (tablets and capsules) are indicated specifically for high blood pressure. Verapamil hydrochloride injection is used in hospital settings for rapid conversion of SVT to normal rhythm and for temporary control of fast heart rates in atrial flutter or atrial fibrillation.
