Hemp and Estradiol

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Hemp interacts with whole categories of medication. Estradiol falls into 3 of those categories, so it can interact with Hemp in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ESTROGENS

1 of 3

Why this applies to Estradiol

Estradiol falls under ESTROGENS — the group Hemp interacts with.

Theoretically, hemp might interfere with hormone therapy due to its estrogenic effects.
In an ovariectomized animal model, a diet containing hemp seed 1%, 2%, or 10% resulted in normalized plasma levels of 17-beta-estradiol. The mechanism of action for this effect is unclear.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the honest picture on hemp and your estradiol. The concern is purely theoretical right now. In one study using animals, adding hemp seed to the diet seemed to nudge natural estrogen levels back toward normal, but researchers couldn't pin down how or why. We don't have human studies showing this actually changes how estradiol works in real life.

In theory, if hemp has any mild estrogen-like activity, it could slightly add to or shift your hormone therapy's effects, things like breast tenderness, spotting, or bloating. But this is weak, lab-and-animal level evidence. Eating hemp seeds as food is very unlikely to matter. If you're using concentrated hemp supplements regularly, just mention it to your pharmacist or doctor so they can keep an eye on things.

Minor

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

2 of 3

Estradiol also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, hemp might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that hemp induces CYP1A2 enzymes.
Severity Minor Likelihood Possible Evidence D

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with hemp and your estradiol. Estradiol is the active estrogen itself (not a prodrug that needs switching on), and one of the enzymes that helps clear it from your body is called CYP1A2. Lab studies suggest hemp may speed that enzyme up a bit. In theory, a faster cleanup could lower your estradiol levels slightly, which might mean your menopause symptoms (vaginal dryness, itching, burning, or hot flashes) aren't quite as well controlled.

That said, I'd file this under interesting but not alarming. The evidence is only from test-tube and animal work, not real people, and the rating is minor. Food amounts of hemp seeds or oil are very unlikely to cause trouble. If you're using concentrated hemp supplements daily and notice symptoms creeping back, just mention it to your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Estradiol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, hemp might decrease the levels and clinical effects of CYP3A4 substrates.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for estradiol: your body relies on an enzyme called CYP3A4 to break it down. Some early lab research hints that hemp may speed that enzyme up. If that happened in real life, your body could clear estradiol a little faster, which might soften how well it works (think a return of menopause-type symptoms like vaginal dryness, itching, or hot flashes).

The reassuring part: this is only test-tube evidence, rated as a minor and theoretical interaction. We don't have human studies showing it actually matters, and everyday hemp foods like seeds or oil are very unlikely to cause trouble. If you're using concentrated hemp supplements regularly, just mention it to your pharmacist or doctor so they can keep an eye on your symptoms.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

These ratings are for information only. Never start, stop, or otherwise change your therapy before speaking with your provider.

From our pharmacist’s overview on the Estradiol page

“Estradiol (Estrace, Vivelle, Vagifem, and other brands) is estrogen therapy, and its interaction list deserves careful reading, especially the three entries rated major. Grapefruit can push estradiol levels higher, St.”

About Hemp

What is Hemp?

Hemp
  • Plant-based protein source
  • Heart health support
  • Skin and hair health
  • Reducing inflammation
  • General nutrition (omega fatty acids)

Hemp seeds and hemp seed oil are nutritious foods rich in protein, fiber, and healthy omega-3 and omega-6 fatty acids, and they are generally safe for most people as part of the diet. While the nutrition is real, strong human evidence that hemp supplements treat specific medical conditions is limited. Talk with your pharmacist or doctor before using hemp products for a health concern, especially if you are pregnant or breastfeeding.
